to the Vice President of Clinical Operations, the Nurse Practitioner (NP), will
be completing in:home Health Risk Assessments (HRA) on a tablet device for
members of health plans with which Ingenios Health is contracted. NP's will
complete a comprehensive assessment at each member's home, lasting 45 minutes
on average, in accordance with State and Federal rules, regulations, and
nursing standards of care in all areas of the assessment, which typically
includes: identification of all current chronic conditions and completion of
related care plan, a medical history review, simple physical examination,
medication review/reconciliation, psychological/financial needs assessment,
health and wellness education, and case management and/or other referrals when
appropriate. An HRA visit might also include the use of advanced portable
diagnostic tools and/or in:home labs leading to the early diagnosis of chronic

Practitioners must be willing to complete HRAs in member homes independently.
On rare occasion, a NP may be temporarily positioned in a clinical center
(urgent care clinic, community center, etc.) to complete HRAs.
Note: This position requires a minimum of 2 days per week availability.


Function independently to perform HRAs at an efficient
level of productivity (based on hours worked per week) in member homes

Thoroughly assess and document member health data via
electronic platform to ensure the highest quality HRA is submitted at the end
of every day

Facilitate relationships with members by completing
confirmation calls, honoring reasonable scheduling requests and providing
exquisite customer service

Collaborate effectively with leadership in day:to:day
operations, training/education and administrative processes

Complete related daily duties, to include: correcting
documentation errors on forms and resubmitting, submitting visit support
documents, and electronically signing all completed HRAs.

Other duties as assigned by VP of Clinical Operations

ideal candidate will possess the following:

Graduate of an accredited Nurse Practitioner Program

Current Advanced Practice Registered Nurse (APRN)
Licensure in the state of service

Excellent communication and organizational skills

Willingness to adapt and diversify one's self to
demands of a fast:paced company
