Must be comfortable counseling clients about sexual and drug risk reduction strategies. Must use plan of care that focus on the clients strengths. Must be comfortable assisting clients in a social crisis situation. Must be comfortable providing outreach activities as a means of providing care and education to the public. Must have knowledge of community resources available in Marion County.

Bachelors degree as a generalist social worker preferred. Experience with computer programs such as: Microsoft Office, Outlook, Excel and Word preferred.

Duties and Responsibilities

This is professional position to provide case management to clients receiving clinical care within the Florida Department of Health Marion County. This position allows access to clients medical records and HMS.

Provides ongoing risk assessments to high risk clients receiving clinical care within the Florida Department of Health Marion County HIV Clinic to encourage risk reduction strategies. Clients will be assisted in developing an individualized plan of care by developing their strengths and utilizing their support system to reach optimal mental and physical health.
Assesses clients for economic, social, and physical needs and provides necessary referrals.

Provides direct care coordination services to enhance the participants ability to access care and maintain continuity of care including follow up of referrals and missed appointments. Assesses clients in crisis and provides referral if necessary to higher level of psychiatric care. Manages schedule of assigned clients.
